2011 Lokomotiv Yaroslavl air disaster

7 September 2011

A Yakovlev Yak-42 charter airplane carrying the ice hockey team Lokomotiv Yaroslavl to a season-opening match of the Kontinental Hockey League in Minsk crashed shortly after takeoff from Yaroslavl, Russia. All but one of the 45 people aboard died, among them almost the whole team of players and coaches, including several international stars such as Pavol Demitra, Ruslan Salei, Stefan Liv and head coach Brad McCrimmon. Investigators concluded that the pilots' errors, including having a foot on the brake pedal during the takeoff run, prevented the plane from reaching flying speed, and found deficiencies in crew training. The club sat out the season, was rebuilt with new players and returned in 2012, and the tragedy led to stricter rules for charter aviation.
